Students Needing Medication at School
It is a Department of Education requirement that any medication administered at school, (including asthma preventatives or relievers) must first be authorised in writing by the parent/carer. Staff will not administer any medication (short or long term) without this written authorisation. Parents/Carers are asked not to make requests to the class teacher to administer.
Parents/Carers must provide the medication to the school office in a pharmacy labelled bottles/boxes and complete the necessary authorisation form. This form is available in the school office or on the school wesbite at: http://www.willandraps.wa.edu.au/medical-forms.html
